Referral Process

Referrals for residency are made by referring organizations completing a referral packet that includes all of the required information about the resident including documentation of homelessness and HIV positive status. Potential residents are referred to Project Lazarus by hospitals, homeless shelters, case managers and other means, followed by an appointment with the Clinical Team in which the individual is asked a series of questions to determine his/her appropriateness for residency. If the individual is deemed appropriate, residency begins immediately upon a vacancy.
